Memory Care Residences at Creekstone

The Memory Care Residences at Creekstone is Rochester Presbyterian Home’s third Eldercare community guided by the principles of the Eden Alternative®. Creating an enriching, engaging, family environment through dignity, respect and participation, SWBR designed the community, which cares for Elders with early- to mid-stage dementia.

Four L-shaped, ranch-style homes form a secure, communal courtyard with dining patios, walking paths, planting beds, and shading arbor. A screened in front porch offers another outdoor option allowing Elders to watch and engage in the activity of their community. Inside, the heart of each home is the living room with hearth, large dining room and state-of-the-art kitchen. Off the main living area are fourteen private bedrooms with private baths.

This model has proven particularly comforting for persons living with dementia. The small home setting is familiar and reassuring. The person-centered care philosophy embraced and provided by the staff assures that the Elders are loved as family.
